If your A&D blood pressure monitor or medical equipment is in need of servicing or recalibration, please contact our customer service team on 1800 011 112 and complete our A&D Declaration Form on this page when sending your A&D Medical product to our Head Office.

 

 

A&D Declaration form for all Recalibrations, Service & Repairs

IMPORTANT REMINDER: We require this Service Declaration Form to be completed and returned to us, whenever you are requesting a recalibration or service repair.

This new procedure will enable us to access and service calibrations and repairs in a timely manner, especially if there is an intermittent fault, and return the item back to you promptly.
